« Previous Product Next Product »The Toby Tracheasaurus Speech Therapy Forms are designed to assist children through the process of tracheotomy surgery and speech therapy afterwards by using a dinosaur illustration (who has had a tracheostomy) to relate their experiences. The forms include an Appointment Card ($4 per pad of 25 cards) to help children remember their appointments; a Therapy Agreement ($6 per pad of 25) to help the child set goals between the child and the therapist; Homework forms ($6 per pad of 25) that serve as a checklist for homework activities for speech therapy; Progress Chart ($6 per pad of 25) to help the child, the family, and the therapist monitor therapy goals in a way that is fun and motivating for the child; an Award ($8 per pad of 25) which is illustrated with Toby Tracheasaurus and friends to congratulate the child for meeting therapy goals and working hard; and Chart Labels ($8 per package of 20) which are stickers for attachment to the child's hospital chart or headboard to alert medical staff to the communication orders for the child. DIMENSIONS: Appointment Cards and Chart Labels are 3 x 5 inches; all other forms are 8.5 x 11 inches. COLOR: Astro Brite Fluorescent green.
