    Ages & Stages Questionnaires, Third Edition (ASQ-3) is a developmental assessment system designed to screen infants and young children for developmental delays during the first five years of life. Parents complete the illustrated 30-item questionnaires at designated intervals, assessing children in their natural environments to ensure valid results. Each questionnaire can be completed in 10 to 15 minutes and covers five key developmental areas: communication, gross motor, fine motor, problem solving, and personal-social. Professionals convert parents' responses of yes, sometimes, and not yet to color-coded scoring sheets, enabling them to determine a child's progress in each developmental area. The ASQ-3 User's Guide offers step-by-step guidance on administering and scoring the questionnaires, setting up a screening system, working with families effectively, and using the system across a wide range of settings. A starter kit includes 21 color-coded, photocopiable questionnaires for use at 2, 4, 6, 8, 9, 10, 12, 14, 16, 18, 20, 22, 24, 27, 30, 33, 36, 42, 48, 54, and 60 months of age; 21 photocopiable, age-appropriate scoring sheets (one for each questionnaire); an ASQ-3 User's Guide; a laminated ASQ-3 Quick Start Guide; and one storage box. Starter kits are available in English, Spanish, or French; however, the French kit includes the 19 questionnaires of the second edition only.     The Choice Switch and Latch Timer (SLAT) is a combination switch latch and switch timer designed to enable people with upper extremity disabilities to operate two switch-adapted games, toys or appliances. The unit enables one user to operate two items in sequence. The unit features two switch inputs, a timer, and control mode selection switch. The switch may operate as a latch switch, for timed seconds or minutes, with both appliances operating in the same mode. Once one item has been activated, the other will not function until the first has been turned off (in latch mode) or the time on the timer has elapsed.

    The Tumble Forms Biform Wedge is a dual-height positioning wedge designed to allow positioning a child at either a higher or lower elevation. Elevation can be changed by simply turning the wedge over. The wedges may be nested together and secured with Velcro straps to expand positioning options. They feature lateral supports and a special contour shape to provide extra support to the user. These wedges are constructed of high density foam with a flexible coating that is washable, seamless, impervious to urine, stain-resistant, and non-toxic. DIMENSIONS: Models 2805A and 2805B have an 18 x 22 inch base with 4 or 6 and 8 or 10 inch elevations, respectively. Models 2805C and 2805D have a 22 x 26 inch base with a 6 or 8 and 10 or 12 inch elevation, respectively. WARRANTY: One year.
